Where and what kind of activities?
If by adventure you mean caving, exploring underground rivers, hiking, rappelling, and zip lining then the link I’m providing you with will lead you there. This place is fantastic and I’ve heard nothing but good things about them and the services they provide. http://www.aventuraspr.com/
If you want to explore some of the worlds best diving then try Desecheo, it’s an Island off of Rincon and it sits very close to the deepest trench in the Atlantic Ocean. The visibility is spectacular, the coral is untouched and there are tons of fish. Here is a link to info on the island as well as info for dive trips.

OK, It is spell PUERTO RICO. And to answer the question;
1st) Puerto Rico has one of the 28 finalist for the new 7 Natural Wonders of the world El Yunque (Which is ranked up with the Great Barrier Reef, Vesuvius, The Grand Canyon, Etc.) I Highly recommend, if you are being accompanied, to stay 2 days in the RAIN FORREST INN (Google their web page for availability). The Rain Forrest is great for hikers, history buffs and has romantic scenery and waterfalls.
2nd) Old San Juan. The oldest standing town in the US. It has night clubs, hotels, Hard Rock Cafe (A Must). And the two historic forts. EL Morro Fort and San Cristobal Fort. Both rich in beauty and history! Also the restaurants have great variety and exotic flavor.
3rd) If you are a surfer and have a board head to Rincon! The surf spot of the island, if you are lucky you may be there for the professional surf competition called La Bruja (The Witch). Named after the local urban legend of the Sea Witch that keeps hold of the young brave men who fail the seas test!
4th) Isla Verde for night life, casinos and a very large and beautiful Moon shaped beach.
5th) Midnight Ca-yak at Moon Lagoon in Fajardo, you can buy tickets at local hotels and online.
